USA (Kentucky) Telephonic Survey

We are Kentucky; USA based organization looking for expert vendor for Conducting Telephonic Survey.
[A] Budget: Looking for Proposals
[B] Scope of Service:
(1) Contractor needs to conduct a survey of residents (general public) to determine their overall perception across the state.
(2) The survey can be conducted by mail or telephone but must cover the entire state.
(3) Contractor will provide staff and facilities to conduct survey, develop a survey instrument, and statistically analyze and interpret survey data.
[C] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office location.
